Exciting opportunity for a seasoned L.M.S.W., L.C.S.W. or L.M.H.C. to join our Behavioral Health Team located at The Nest Community Health Center. URAM-Harlem United’s Wellness Center is a Licensed State Operated Article 31 Mental Health Clinic Collocated in our Federally Qualified Health Center (F.Q.H.C.). Join our integrated health care center providing diagnosis based mental health treatment / psychotherapy within a community based health care center with a strong commitment to our community, team collaboration and excellence. Under the supervision of the Clinical Director of Behavioral Health Services and in accordance with established policies and procedures, the psychotherapist provides assessments, care planning, and psychotherapy to clients referred to from the greater community and provider of our Community Health Center our Article 31 clinic. This position functions in a primarily electronic environment and requires proficiency in ‘Windows” based technology.


The following duties are mandatory requirements of the job:
• Conducting assessments and diagnoses for new and current FQHC clients

• Provide individual psychotherapy to individuals with mental illness, Substance use issues and chronic health conditions including HIV/AIDS using Motivational Interviewing, Cognitive Behavioral Therapy and Harm Reduction principles

• Establishing treatment goals and objectives for and with clients, as part of the interdisciplinary care planning process

• Provide crisis intervention services and if necessary, refer clients for psychiatric assessment

• Facilitate therapy groups weekly

• Assumes role of primary psychotherapist for assigned clients, which includes documenting all sessions for assigned clients

• Insure the integrity and confidentiality of all patient information in the EHR by not sharing personal user name or password with other staff

• Insure the integrity and confidentiality of all patient information in the EHR by exiting the application or locking the system when away from the computer

• Actively participate in regular individual and group supervision and any other team meetings and trainings as assigned

• Collaborate with multidisciplinary team to coordinate care, treatment planning and service delivery to clients

• Coordinate and facilitate case conferences for providers across agency serving same client, for high risk cases and following incidents


• Master’s in Social Work plus LCSW preferred; or LMSW or LMHS with at least two years experience providing psychotherapy
• Experience providing psychotherapy in an Article 31 or similar outpatient clinic required
• Experience working with individuals with history of substance use, mental illness, chronic homelessness and/or HIV/AIDS and other chronic health conditions required
• Experience working with adolescents, young adults and families desired
• Experience facilitating therapy groups desired
• Prefer candidates already credentialed with insurance panels
• Bilingual in Spanish a plus
