Brief Description of Duties:Complete medical history/physical examination on all patients and provide appropriate documentation. Order indicated laboratory and diagnostic tests; collect specimen and perform phlebotomy, as necessary. Review patients� diagnostic test results, and document follow-up for all abnormal results as per SHC policies and procedures. Provide appropriate interventions in the form of prescription and non-prescription drugs, education, referrals and follow-up care. Refer for sub-specialty medical and surgical evaluations as indicated. Communicate with consultants (medical sub specialties, etc.) and provide primary care services based on consultant�s treatment recommendations. Provide appropriate interventions in the form of prescription and non-prescription drugs, education, referral and follow-up care. Participate in the development and implementation of multi-disciplinary case management plans, including identifying and referring patients for treatment of mental health, substance abuse and adherence problems.

Participate in the development, implementation and coordination of the clinical and educational projects with the STAR Program, including providing academic and clinical supervision to trainees and provide on-site training to providers at primary care sites throughout Brooklyn; Academic teaching within the Department of Medicine.

Participate in multidisciplinary case conference rounds.

Be familiar with all relevant city, state and federal policies which impact on the provision of care to HIV-infected patients and maintain standards of medicine and incorporate the policies and procedures of STAR Health Center into practice.

Promote Outreach activities that would help to recruit new patients and maintain patients in care, which will benefit the research priorities of the Department of Medicine.DUTIES AND RESPONSIBILITIES ARE NOT LIMITED TO THE ABOVE POSITION DESCRIPTION

All successful candidates must undergo various background checks, maintain credentials required for continued employment and adhere to the SUNY-DMC UHB Principles of Behavior.

Clinical Faculty and Allied Health professionals must receive and maintain Medical Board authorization.

Qualifications:REQUIRED: M.D. with board certification or board eligibility in Infectious Diseases, Internal Medicine or Family Practice. NYS License. Willingness to work as a member of team of multidisciplinary healthcare providers to provide primary care to HIV & HCV infected individuals Effective written, verbal and interpersonal skills. Basic computer skills, including word processing and data entry. PREFERRED: A minimum of two years of clinical experience in an HIV ambulatory care environment. Multicultural awareness and bilingual language skills (French/Creole) strongly preferred.
